About this role
You will provide speech-language pathology services in a long-term care setting, working days on a 5x8 schedule. This is a 91-day assignment beginning May 18, 2026, with Tide Health Group - Community Care Center of Hondo.
Your responsibilities include:
- Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
- Develop and implement plans of care following all regulatory and clinical practice standards
- Utilize standardized assessments and evidence-based practice to support clinical interventions
- Assess patient progress and adjust treatments accordingly, documenting in clinical records
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
- Provide consultation and counseling to patients, families, caregivers, and other service providers
- Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
- Provide comprehensive discharge summaries per regulatory and clinical requirements
- Document all required information in the electronic documentation system and maintain accurate patient records
- Record treatment minutes for services provided per regulatory requirements
- Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
- Attend facility and company required trainings, in-services, and conferences
- Obtain necessary DME or supplies per regulatory and clinical practice requirements
- Supervise and support CFY or Student Speech-Language Pathologists when needed
- Maintain adherence to ASHA and State required practice acts, code of conduct, and ethics
Requirements: Texas State License required. CCC certification required. Minimum 1 year of SLP experience required. LTC or Rehab experience required. New graduates acceptable with Level II experience. Three years of experience holding active CCS preferred. No on-call or weekend requirements.
Weekly pay is $2,534.40.
