About this role

Tide Health Group - Thunder Care and Rehab is seeking a Speech-Language Pathologist for a long-term care position in Moore, Oklahoma. This is a 91-day assignment working days, 5 days per week, 8 hours per shift. Weekly pay is $2,534.40, with a start date of July 27, 2026.

Your Responsibilities:

  • Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
  • Develop and implement appropriate plans of care following all regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to support clinical interventions
  • Assess patient progress and adjust treatments accordingly; document all findings in the clinical record per regulatory requirements
  • Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation results
  • Make referrals in relation to treatment outcomes
  • Provide consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders
  • Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans
  • Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements
  • Document all required information in the electronic documentation system and maintain patient records according to company policy
  • Accurately record treatment minutes for services provided per all regulatory and clinical requirements
  • Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
  • Participate in facility and company required trainings, in-services, and conferences
  • Obtain necessary DME or supplies per regulatory and clinical practice requirements
  • Supervise and support CFY or Student Speech-Language Pathologists as needed
  • Adhere to ASHA and state required practice acts, code of conduct, and ethics during professional practice

Requirements:

  • Master's degree or higher in Speech-Language Pathology
  • Minimum 1 year of SLP experience required
  • Long-term care or rehabilitation experience required
  • CCC certified SLP required
  • Active ASHA membership and Oklahoma state licensure required
  • No on-call or weekend requirements
