PT - Physical Therapist

Job Details:

• Perform comprehensive patient evaluations including obtaining medical history and conducting systems reviews
• Develop individualized treatment plans with patient-centered goals to reduce pain and restore function
• Conduct ongoing patient reassessments and monitor response to treatment
• Supervise and delegate to ancillary staff and students in accordance with state laws
• Complete accurate documentation within established timeframes per department policy
• Communicate effectively with patients, families, and healthcare team members using AIDET principles
• Provide patient and family education based on assessed educational needs
• Participate in professional development through educational programs and current literature review
• Actively engage in quality improvement activities
• Maintain safe environment for patients, visitors, and staff
• Function within scope of practice as defined by state practice act
• 2 years of physical therapy experience in long-term care setting required
• Texas license or compact state license required
• No weekend coverage required
• No on-call responsibilities
