RN - Case Management

Job Details:

• Provide comprehensive care coordination for assigned patients in an acute adult care setting at a Level I Trauma Center with 600+ beds
• Schedule varies and includes rotating weekends
• No on-call requirement
• Consistent weekly work schedule
• Manage individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes
• Collaboratively develop and manage interdisciplinary patient discharge plans and communicate effectively across the continuum of care
• Evaluate medical necessity and appropriateness of care, assessing patient admissions and continued stay utilizing standard criteria
• Identify and resolve issues that may delay patient discharge
• Establish and promote collaborative relationships with physicians, payers, and other members of the health care team
• Educate internal health care team members on case management and managed care concepts
• Must have recent acute care experience exceeding 2 years in Adult Health case management
• Must possess knowledge of care management, acute care environments, community resources, and utilization management
• Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills
• Current Registered Nurse (RN) license required
• Basic Life Support (BLS) certification required
• CCM (Certified Case Manager) certification preferred
• Bachelor's degree in case management or health care preferred
• 3-5 years of clinical experience required
