Occupational Therapist Job in Grand Junction, CO

You will deliver occupational therapy services across multiple settings in the Grand Junction area, including a therapy gym, day center, and patient homes. This is a full-time position working Monday through Friday, 8:00 AM to 5:00 PM, for 91 days starting October 21, 2026.

Your responsibilities:

• Evaluate and develop individualized occupational therapy treatment plans for patients
• Conduct home-based safety assessments to help participants remain safely in their homes and communities
• Provide skilled therapeutic interventions in the day center setting
• Coordinate durable medical equipment and supportive services to promote patient independence
• Help improve patient strength, mobility, daily living skills, and confidence
• Collaborate with physicians, nurses, therapists, social workers, and other interdisciplinary team members
• Travel locally between work settings as needed

Requirements:

• Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program
• Current NBCOT registration
• Current Colorado Occupational Therapist license
• 2–3 years of occupational therapy experience
• Experience working with older adults or geriatric populations
