Your Responsibilities:

• Provide physical therapy services to patients in the post-acute care setting
• Work with patients during rehabilitation and recovery phases
• Develop and implement individualized treatment plans focused on optimal patient outcomes
• Document patient progress accurately and thoroughly
• Coordinate and communicate with interdisciplinary healthcare team members
• Monitor patient progress and adjust treatment plans as needed

What We're Looking For:

• Current Physical Therapist credential
• Valid state licensure in California
• Experience in post-acute or rehabilitation care preferred
• Strong communication and documentation skills
• Ability to work collaboratively with clinical teams
