About this role

Tide Health Group - Purcell Care Center is hiring a Physical Therapist for a temporary position in long-term care. You'll work days, 5 days per week on an 8-hour schedule for 91 days, starting June 15, 2026. Weekly pay is $2,390.40.

Your responsibilities include:

  • Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care
  • Develop appropriate plans of care following all regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to support clinical interventions
  • Administer various rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
  • Assess patient progress and adjust treatments accordingly in clinical records per regulatory requirements
  • Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Communicate effectively with supervisor and health team members regarding patient progress and treatment plans
  • Provide comprehensive discharge summaries per regulatory and clinical requirements
  • Document all required information in electronic documentation system and maintain patient records
  • Accurately record treatment minutes for services provided per regulatory requirements
  • Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
  • Attend facility and company required trainings, in-services, and conferences
  • Obtain necessary DME or supplies per regulatory and clinical practice requirements
  • Supervise and support physical therapist assistants, rehabilitation aides, and/or students
  • Maintain adherence to APTA and State Required practice acts, code of conduct, and ethics

Requirements:

  • 1 year of PT experience (new graduates accepted with Level II experience)
  • Long-term care or rehabilitation experience required
  • Oklahoma state license required
  • No on-call or weekend requirements
