About this role
Tide Health Group - Purcell Care Center is hiring a Physical Therapist for a temporary position in long-term care. You'll work days, 5 days per week on an 8-hour schedule for 91 days, starting June 15, 2026. Weekly pay is $2,390.40.
Your responsibilities include:
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care
- Develop appropriate plans of care following all regulatory and clinical practice standards
- Utilize standardized assessments and evidence-based practice to support clinical interventions
- Administer various rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
- Assess patient progress and adjust treatments accordingly in clinical records per regulatory requirements
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
- Provide consultation and counseling to patients, families, caregivers, and other service providers
- Communicate effectively with supervisor and health team members regarding patient progress and treatment plans
- Provide comprehensive discharge summaries per regulatory and clinical requirements
- Document all required information in electronic documentation system and maintain patient records
- Accurately record treatment minutes for services provided per regulatory requirements
- Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
- Attend facility and company required trainings, in-services, and conferences
- Obtain necessary DME or supplies per regulatory and clinical practice requirements
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students
- Maintain adherence to APTA and State Required practice acts, code of conduct, and ethics
Requirements:
- 1 year of PT experience (new graduates accepted with Level II experience)
- Long-term care or rehabilitation experience required
- Oklahoma state license required
- No on-call or weekend requirements
