Physical Therapist Job in Newport, VT

Orleans Essex Visiting Nurses Association & Hospice, Inc. is seeking a Physical Therapist for home health and hospice services in Newport, VT. You'll provide direct patient care across a variety of settings, traveling between patient locations to deliver evidence-based therapy services.

Position Details:

• Monday through Friday, 8:00 AM to 5:00 PM
• 91-day assignment beginning September 28, 2026
• Weekly pay: $2,551
• Home health and hospice-based therapy delivery

Your Responsibilities:

• Conduct patient assessments and develop individualized treatment plans
• Provide outpatient physical therapy services in patients' homes
• Travel between patient locations for home-based therapy visits
• Document care using the HealthWyse EMR system
• Manage schedule adjustments and respond to changing patient needs
• Provide holiday coverage as needed

Required Qualifications:

• Current Physical Therapist license or degree
• Demonstrated experience with OASIS assessments
• Proven background in home health and hospice care
• Ability to travel efficiently between patient locations
• Flexibility and comfort managing dynamic schedules
