PT - Physical Therapist

Job Details:

• 13-week contract with 40 guaranteed hours per week
• Monday-Friday schedule from 8:00 AM to 5:30 PM
• Rotate one Saturday every 6-8 weeks (receive weekday off when working Saturday)
• No holiday requirements
• Occasional travel to central/northern regions with mileage reimbursed at government approved rate
• Must live outside 50-mile radius from facility
• Master's Degree in Physical Therapy required
• Home health experience required with adult/geriatric population
• Experience with orthopedic, neurological and functional mobility diagnoses
• Wisconsin Physical Therapy license required
• NPI number required (must begin application process if not already obtained)
• AHA or ARC BLS certification required
• Use laptop for point of service documentation with NetSmart EMR system
• Cell phone provided for GPS and communication
• Expected to average 4.5 patient visits per day
• Evaluate, treat and plan patient care according to Physical Therapy standards
• Document to meet regulatory and reimbursement requirements
• Business casual or scrubs acceptable
• Must wear mask at all times during flu season if declining flu and COVID vaccination
