PT - Outpatient

Job Details:

• Plan and execute rehabilitation programs to help patients return to higher levels of physical function
• Utilize therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques
• Apply knowledge of anatomy, exercise physiology, biomechanics, kinesiology, neuroscience, and pharmacology
• Develop individualized Plans of Care based on initial and ongoing evaluations with short and long-term goals
• Evaluate patient mobility, strength, and current functional levels while setting realistic, attainable goals
• Implement consistent treatment schedules and adjust plans according to patient progress
• Document patient progress according to approved medical documentation procedures
• Maintain patient records ensuring compliance with third party payer requirements
• Perform initial and ongoing physical therapy evaluations according to APTA and state standards
• Supervise physical therapy assistants in a professional manner
• Maintain open communication with therapy staff, department staff, and referral sources
• No on-call responsibilities required
• No weekend work required
• 1+ years of physical therapy experience required
• Outpatient experience required
• Must live outside 50-mile radius of facility
• Required certifications: BLS (AHA)
• Illinois Physical Therapist License required
• Current skills checklist within one year required
