Physical Therapist Job in Sunnyvale, CA

Pathways Home Health & Hospice is seeking a Physical Therapist to deliver skilled therapy services to patients throughout the San Mateo Peninsula region. You will work 5 days per week on day shifts for a 91-day assignment beginning May 27, 2026, earning $2,920 weekly.

Your responsibilities will include:

• Evaluate and treat patients using medically prescribed physical therapy programs
• Restore function to prevent or minimize disability following disease or injury
• Help clients reach maximum performance and function within the limits of their capabilities
• Make initial admission and evaluation visits
• Supervise Physical Therapy Assistants when applicable
• Document observations, clinical findings, skilled interventions, treatments, goals, and discharge plans
• Notify the assigned Clinical Team Manager of OASIS scores within established timelines
• Travel to multiple cities within your assigned territory, including South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Carlos, Foster City, San Mateo, Redwood City, Emerald Hills, Menlo Park, Mountain View, and neighboring areas as needed

Qualifications:

• Physical Therapist degree required
• One year of home health experience required
• Ability to perform duties consistent with skills and training for this position
