Physical Therapist Job in Lakeport, CA

Sutter Health Care at Home Lakeside is seeking a Physical Therapist for home health services in Lakeport, CA. This is a 91-day travel assignment with a 5x8 Days schedule, paying $2,812.32 weekly.

Role Responsibilities:

• Provide home health physical therapy services with approximately 5 weighted visits per day (4–5 patients daily)
• Conduct comprehensive physical therapy assessments and treatments
• Address gait training, therapeutic exercises, patient education, and discharge planning
• Work with adult and geriatric patient populations
• Treat various conditions including orthopedic injuries, neurological disorders, post-operative rehabilitation, and chronic pain management

Requirements:

• Minimum 1 year of home health physical therapy experience
• Current California physical therapy license
• Valid California driver's license and active auto insurance
• Nursys verification must be current within the last 2 weeks
• BLS certification preferred
• Epic charting system experience preferred

Important Details:

• No float or call requirements
• 40 hours of orientation required (non-billable; modules average 1–5 hours)
• Must be available for lab work within 3–5 days of contract signing
• All certifications must be AHA or American Red Cross
