PT - Physical Therapist

Job Details:

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care
• Develop appropriate plan of care following all regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Administer various procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments accordingly in clinical records per regulatory requirements
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers and other service providers
• Communicate effectively with supervisor and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in electronic documentation system and maintain patient records
• Accurately record treatment minutes for services provided per regulatory requirements
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Attend facility and company required trainings, in-services and conferences
• Obtain necessary DME or supplies per regulatory requirements
• Supervise and support physical therapist assistants, rehabilitation aides and/or students
• Adhere to APTA and State Required practice acts, code of conduct, and ethics
• 1 year of PT experience required (new grads acceptable with Level II experience)
• Long-term care or rehabilitation experience required
• Massachusetts State License required
• No on-call or weekend requirements
