About this role

Tide Health Group - Plymouth Rehabilitation & Health Care Center is seeking a Speech-Language Pathologist for a long-term care assignment in Plymouth, MA. This is a full-time position working 5 days per week, 8 hours per day on the day shift, with a start date of June 22, 2026. The assignment is for 91 days.

Your Responsibilities:

  • Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
  • Develop appropriate care plans following all regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to support clinical interventions
  • Assess patient progress and adjust treatments accordingly, documenting in clinical records
  • Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Communicate effectively with supervisor and health team members regarding patient progress and treatment plans
  • Provide comprehensive discharge summaries per regulatory and clinical requirements
  • Document all required information in the electronic documentation system and maintain patient records
  • Record treatment minutes for services provided per regulatory requirements
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Attend facility and company required trainings, in-services, and conferences
  • Obtain necessary DME or supplies per regulatory and clinical practice requirements
  • Supervise and support CFY or Student Speech-Language Pathologists

Requirements:

  • Master's degree in Speech-Language Pathology
  • 1 year of SLP experience required
  • Long-term care or rehabilitation experience required
  • Massachusetts state license required
  • CCC certified SLP required
  • Active ASHA membership and state licensure required
  • New graduates accepted with Level II clinical experience
