About this role
Tide Health Group - Plymouth Rehabilitation & Health Care Center is seeking a Speech-Language Pathologist for a long-term care assignment in Plymouth, MA. This is a full-time position working 5 days per week, 8 hours per day on the day shift, with a start date of June 22, 2026. The assignment is for 91 days.
Your Responsibilities:
- Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
- Develop appropriate care plans following all regulatory and clinical practice standards
- Utilize standardized assessments and evidence-based practice to support clinical interventions
- Assess patient progress and adjust treatments accordingly, documenting in clinical records
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
- Provide consultation and counseling to patients, families, caregivers, and other service providers
- Communicate effectively with supervisor and health team members regarding patient progress and treatment plans
- Provide comprehensive discharge summaries per regulatory and clinical requirements
- Document all required information in the electronic documentation system and maintain patient records
- Record treatment minutes for services provided per regulatory requirements
- Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
- Attend facility and company required trainings, in-services, and conferences
- Obtain necessary DME or supplies per regulatory and clinical practice requirements
- Supervise and support CFY or Student Speech-Language Pathologists
Requirements:
- Master's degree in Speech-Language Pathology
- 1 year of SLP experience required
- Long-term care or rehabilitation experience required
- Massachusetts state license required
- CCC certified SLP required
- Active ASHA membership and state licensure required
- New graduates accepted with Level II clinical experience
