Job Details:

• Work up to 40 hours per week providing occupational therapy services
• Provide in-home therapy services to home care and hospice patients
• Work independently while collaborating with multidisciplinary team members
• Focus on maximizing patient independence and safety in home environment
• Vermont occupational therapy licensure required
• Valid driver's license and reliable vehicle required for travel to patient homes
• Home care setting experience preferred
• Must demonstrate strong independent work skills and collaborative team approach
