SLP Job in Moore, OK

Tide Health Group - Thunder Care and Rehab is seeking a Speech-Language Pathologist (SLP) for a long-term care position in Moore, Oklahoma. This is a day shift role working 5x8 hours per week for 91 days, starting July 6, 2026.

Key Responsibilities:

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
• Develop and implement plans of care that follow all regulatory and clinical practice standards
• Use standardized assessments and evidence-based practice to guide clinical interventions
• Monitor patient progress and adjust treatment plans as needed
• Evaluate treatment outcomes for effectiveness and modify services based on patient response
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate with supervisor and health team members regarding patient progress and care plans
• Prepare comprehensive discharge summaries per regulatory requirements
• Document all information in the electronic records system and maintain accurate patient records
• Record treatment minutes for services provided per regulatory standards
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Attend required facility and company trainings, in-services, and conferences
• Obtain necessary DME or supplies as clinically indicated
• Supervise and support CFY or student speech-language pathologists

Qualifications:

• Therapy degree (SLP certification)
• Minimum 1 year of SLP experience
• Long-term care or rehabilitation experience required
• Oklahoma State License required
• CCC certification required
• Active ASHA membership and State Licensure required
• New graduates accepted with Level II clinical fellowship experience

Schedule: Days only; no on-call responsibilities or weekend requirements.

Compensation: $2,534.40 weekly.
