Opportunity:

PT - Physical Therapist

Job Details:

• Conduct skilled home health visits with 6-8 patients per day (minimum 6 points per day)
• Work 1-2 weekends per contract as needed
• Self-schedule visits and hours based on office needs
• Must have 1 year of experience and home health experience required
• OASIS training preferred
• Required certifications: BLS and state license
• Must have valid driver's license and personal vehicle for home visits
• Dress code: Any solid color scrub
• Provided equipment: Laptop, MiFi for internet access, portable printers
• Documentation systems: BayCare Forms, BayCare Scheduling, Ace Timesheet
• Work with patients including med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory patients, neuro, trachs, trauma, amputees
• Provide services for Medicare, Medicare Advantage and Commercial insurance patients
• Skills required: Assessment, cardiopulmonary, documentation, equipment and devices, ortho, neuro, home exercise programs, medications, patient/family education, safety and injury prevention, billing
• 40-hour orientation required
• Must complete pre-start modules (up to 4 billable hours for allied professionals)
• Must live at least 100 miles from facility location
