RN - Case Management

Job Details:

• Provide discharge planning support for patients throughout all areas of the hospital
• Perform utilization review and case management functions
• Coordinate discharge planning for inpatient populations, including psychiatric and behavioral health patients
• Collaborate with hospital departments and care teams to support patient transitions and discharge needs
• Assist with identifying and coordinating resources necessary for an appropriate discharge plan
• Support continuity of care throughout the discharge planning process
• Maintain appropriate documentation within EPIC
• This role is not involved in direct bedside patient care
• Patient population includes adolescent, adult, and geriatric age groups
• Inpatient population is 100% with rare ambulatory or outpatient coverage
• Case management staffing goal of 1:20 patient ratio
• On-call responsibilities required
• Float and coverage across all areas of the hospital
• Business casual dress code required
• Experience with EPIC electronic health record system required
