PT - Physical Therapist

Job Details:

• Day shift position working 8:00 AM to 4:30 PM, Monday through Friday
• No weekend or holiday requirements
• Home health care setting serving patients in their homes across multiple counties in West Virginia
• Treat 5-6 patients per day (Start of Care = 2, Discharge = 1, Recertification = 1.5, Regular visit = 1)
• Common diagnoses include general orthopedics (hips, knees), generalized weakness, strengthening, and cardiac rehab
• Coverage area spans up to 8 counties (Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, Pocahontas); ideal coverage in Webster and Nicholas Counties though flexible based on therapist location
• Self-scheduling allowed with flexibility to organize patient visits by area based on initial and reassessment guidelines
• Charting completed in Epic system; positive attitude toward learning Epic documentation required
• Dress code: Navy scrubs or khaki bottoms with a navy polo
• Required certifications: BLS Certification
• Must meet all PT licensure requirements for practicing in the state of West Virginia
• Cancellation policy: 2 shifts per 13-week contract
