<p><strong>Allied Position Job Notes:</strong></p> <p>Travel or Local Allied Healthcare Professional needed for ASAP start. A minimum of 2+ years of recent hospital bedside experience is required. Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Provide comprehensive physical therapy services for PACE (Program of All-inclusive Care for the Elderly) participants<br>• Conduct initial assessments for potential participants and annual assessments for existing participants<br>• Perform assessments when there is a change of condition in participants<br>• Collaborate with interdisciplinary team, participants, and family members to create and implement individualized plans of care<br>• Focus on improving quality of life and maintaining seniors' health so they can live safely in their homes<br>• Provide clinical education and training to PACE staff and volunteers<br>• Maximize safety and functional abilities of participants during activities of daily living<br>• Work in a variety of environments including home-based settings<br>• Graduate from a CAPTE accredited Physical Therapy program required<br>• Currently licensed as a PT by the state of Colorado required<br>• 1 year of PT experience required; 2-3 years of home health or home-based PT experience preferred<br>• Valid driver's license, reliable transportation, and adequate automobile insurance required<br>• Computer literacy in MS Word, Excel, Access, and Outlook required<br>• Current CPR certification required and must be maintained throughout employment<br>• Excellent interpersonal and communication skills required<br>• Ability to communicate effectively in highly emotional interactions</p><p><strong>Facility:</strong></p><p>This healthcare organization operates a PACE program that provides comprehensive, coordinated, and proactive whole-person care for elderly participants.
