Opportunity:

RN - Case Management

Job Details:

• Work 5 days per week, 8-hour day shifts from 8:00 AM to 4:30 PM
• Every other weekend required with 2 two-day weekends in a 6-week rotation
• Every other holiday required
• Float to Medical units, ICU/CCU, and Maternal Child required
• Manage up to 20 patient assignments per day
• Perform inpatient case management including discharge planning and handling large caseloads
• Apply care coordination, continued stay reviews, and utilize InterQual and Milliman guidelines
• Work with patient age groups including infants, adults, and geriatrics
• Experience required in Acute Hospital settings including ICU, MS, SDU/PCU/IMC/Obs, and Women's Services
• Discharge planning and large caseload management experience required
• 2 years of case management experience required
• Travel experience required
• Social Workers (LMSW, MSW, or LCSW) with 2+ years of experience will be considered
• Charting system experience required
• Required certifications: BLS and Maryland license
