Job Details:

• Must have Skilled Nursing experience
• Bachelor's degree required, Master's or Doctorate preferred
• Active Physical Therapist license in Massachusetts required
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care or skilled nursing facility preferred
• Partner with residents and care providers to maximize functional abilities and independence
• Perform evaluative and therapeutic procedures under physician orders
• Evaluate residents per department protocols and establish therapeutic plans of care
• Document all interactions in Electronic Medical Record per department procedures
• Provide consultation to physicians and healthcare team members
• Monitor and evaluate treatment effects and progress treatment programs
• Recommend and obtain appropriate assistive devices with proper instruction
• Provide education to residents and caregivers
• Work collaboratively with Physical Therapist Assistants and other therapy professionals
• Comply with Medicare and other reimbursement regulations
• Supervise assistant-level staff when appropriate
• Provide mentorship to new staff during orientation
• Must be able to lift up to 50 pounds and perform combination of sitting, standing, walking, bending, and reaching
• Strong verbal, written, and presentation communication skills required
• Proficient in Microsoft Office required
