Opportunity:

PT - Physical Therapist

Job Details:

• Conduct 6-8 home skilled visits per day with minimum of 6 points per day
• Self-schedule visits and hours based on home care office needs
• Work every 3rd weekend as required
• Must have 2 years of physical therapy experience
• Home health experience required with OASIS training preferred
• Required certifications: BLS and state physical therapy license
• Must have valid driver's license and personal vehicle for home visits
• Dress code: Any solid color scrubs
• Work with provided equipment including laptop, MiFi for internet access, and portable printers
• Use BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
• Complete 16 hours of orientation
• Two pre-approved time off requests allowed
• Must live at least 100 miles from facility (100-mile radius rule applies)
• Florida license or compact license required at time of submittal
• Treat patients with cardiopulmonary, geriatric, neurological, orthopedic, and oncology conditions
• Provide comprehensive assessment, patient/family education, home exercise programs, and safety training
• Complete compliance documentation 10 days before start date
