Job Details:

• Conduct comprehensive assessments of patients' physical, mental, and emotional needs
• Develop personalized treatment plans to improve functional independence in activities of daily living (ADLs)
• Provide direct therapy interventions including therapeutic exercises, adaptive equipment training, and cognitive training
• Collaborate with healthcare team members including nurses, physicians, and speech therapists to ensure optimal patient care
• Educate patients and families on home safety, adaptive techniques, and equipment
• Document patient progress and treatment plans in compliance with facility and regulatory standards
• Stay current on best practices and new treatment methodologies in occupational therapy
• Degree in Occupational Therapy from an accredited program required
• Valid Occupational Therapy license in California required
• Experience in a skilled nursing facility or geriatric care setting preferred
• Strong knowledge of rehabilitation techniques, equipment, and therapeutic modalities required
• Excellent communication, interpersonal, and organizational skills required
• Ability to work effectively both within a team and independently
