PT - Physical Therapist

Job Details:

• Monday through Friday schedule from 8:00 AM to 5:30 PM
• 40 guaranteed hours per week
• 13-week contract
• Occasional travel to central and northern regions required with mileage reimbursed at government approved rate
• Physical therapists rotate one Saturday every 6-8 weeks with a compensatory day off during the week; no holiday rotation required
• No on-call or call-back responsibilities
• Evaluate, treat, and plan patient care in accordance with Physical Therapy Standards of Practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Document to meet regulatory and reimbursement requirements
• Meet productivity expectations of an average of 4.5 visits per day
• Use laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
• Scrub colors: any color or business casual attire accepted
• Required certifications: AHA or ARC BLS
• Wisconsin state licensure required
• NPI number required; candidates without one must begin the application process prior to start
• Master's Degree in Physical Therapy required
• Minimum experience required includes home health experience and training working with adult and geriatric populations
• Must reside outside a 50-mile radius of the facility
