PT - Physical Therapist

Job Details:

• Work 8 variable day shifts Monday through Friday
• Guaranteed 38 hours per week with flex up to 40 hours
• No holiday rotation required
• Occasional travel to central and northern regions required
• Mileage reimbursed at government approved rate
• Physical therapists rotate one Saturday every 6-8 weeks with a compensatory day off during that week
• May be placed on low census call; call back minimum is 2 hours
• Evaluate, treat, and plan patient care in accordance with Physical Therapy Standards of Practice
• Population served includes adult and geriatric patients with a variety of orthopedic, neurological, and functional mobility diagnoses
• Document to meet regulatory and reimbursement requirements using NetSmart EMR and laptop for point of service documentation
• Cell phone required for GPS and communication
• Meet productivity expectations of an average of 4.5 visits per day
• Bachelor's Degree in Physical Therapy required; Masters Degree preferred
• Minimum experience in home health required with training working with adult and geriatric populations
• Required certifications: AHA or ARC BLS, valid Wisconsin state licensure, valid driver's license with proof of car insurance, and NPI number required
• Scrub colors: any or business casual attire accepted
