Opportunity:

RN - Case Management

Job Details:

• Monday through Friday 8:00 AM to 5:00 PM with some Saturdays required
• 2-3 years of experience in case management required
• First-time travelers accepted
• Travel experience required; local candidates within 75-mile radius not accepted
• Required certifications: RN, BLS
• Work with adult patients needing home health set up and discharge assistance
• Must have strong technology skills including typing, Microsoft Office, and Epic
• Ability to work in a fast-paced environment with strong organizational and prioritization skills required
• Excellent written and verbal communication skills required
• Key skills include care coordination, discharge planning, needs assessment, DME ordering, prior authorizations, and medical necessity review
• Community hospital experience preferred
• LTAC experience preferred
• All travelers will float within service line
