About this role
Tide Health Group – Lanessa Extended Care is seeking a Speech-Language Pathologist for a long-term care position in Webster, MA. This is a full-time role working 5 days per week, 8-hour shifts, with no on-call or weekend requirements.
About the Role
You will screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders in a long-term care setting. Your responsibilities include:
- Develop and implement plans of care aligned with regulatory and clinical practice standards
- Use standardized assessments and evidence-based practice to guide clinical interventions
- Monitor patient progress, adjust treatments as needed, and document outcomes in the clinical record
- Evaluate treatment effectiveness and modify services based on patient evaluation and progress
- Make appropriate referrals based on treatment outcomes
- Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
- Communicate with supervisors and health team members regarding patient progress, barriers, and treatment plans
- Prepare comprehensive discharge summaries per regulatory and clinical requirements
- Maintain accurate documentation in the electronic system and comply with company record-keeping policies
- Record treatment minutes for all services provided per regulatory standards
- Participate in patient care conferences, utilization reviews, and rehabilitation meetings
- Complete required facility and company trainings, in-services, and conferences
- Obtain necessary durable medical equipment or supplies as clinically indicated
- Supervise and support Clinical Fellow Year or student Speech-Language Pathologists as needed
Requirements
- Master's degree in Speech-Language Pathology or related therapy field
- Minimum 1 year of SLP experience (new graduates with Level II clinical experience accepted)
- Long-term care or rehabilitation experience required
- Current Massachusetts state license
- Certificate of Clinical Competence (CCC) from ASHA required
- Active ASHA membership and state licensure required
- Adherence to ASHA code of conduct and ethics
Position Details
Duration: 91 days | Start date: August 31, 2026 | Weekly pay: $2,534.40
