About this role

Tide Health Group – Lanessa Extended Care is seeking a Speech-Language Pathologist for a long-term care position in Webster, MA. This is a full-time role working 5 days per week, 8-hour shifts, with no on-call or weekend requirements.

About the Role

You will screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders in a long-term care setting. Your responsibilities include:

  • Develop and implement plans of care aligned with regulatory and clinical practice standards
  • Use standardized assessments and evidence-based practice to guide clinical interventions
  • Monitor patient progress, adjust treatments as needed, and document outcomes in the clinical record
  • Evaluate treatment effectiveness and modify services based on patient evaluation and progress
  • Make appropriate referrals based on treatment outcomes
  • Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
  • Communicate with supervisors and health team members regarding patient progress, barriers, and treatment plans
  • Prepare comprehensive discharge summaries per regulatory and clinical requirements
  • Maintain accurate documentation in the electronic system and comply with company record-keeping policies
  • Record treatment minutes for all services provided per regulatory standards
  • Participate in patient care conferences, utilization reviews, and rehabilitation meetings
  • Complete required facility and company trainings, in-services, and conferences
  • Obtain necessary durable medical equipment or supplies as clinically indicated
  • Supervise and support Clinical Fellow Year or student Speech-Language Pathologists as needed

Requirements

  • Master's degree in Speech-Language Pathology or related therapy field
  • Minimum 1 year of SLP experience (new graduates with Level II clinical experience accepted)
  • Long-term care or rehabilitation experience required
  • Current Massachusetts state license
  • Certificate of Clinical Competence (CCC) from ASHA required
  • Active ASHA membership and state licensure required
  • Adherence to ASHA code of conduct and ethics

Position Details

Duration: 91 days | Start date: August 31, 2026 | Weekly pay: $2,534.40
