Job Details:

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
• Develop appropriate care plans following regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice for clinical interventions
• Provide rehabilitative, skilled and medically necessary treatment interventions
• Administer various procedures including ADL retraining, therapeutic activities, therapeutic exercises, and modalities
• Use supportive and assistive devices as part of rehabilitation plans
• Assess patient progress and adjust treatments accordingly
• Evaluate treatment outcomes for effectiveness and modify services as needed
• Provide consultation and counseling to patients, families, caregivers and service providers
• Communicate effectively with supervisors and health team members regarding patient progress
• Provide comprehensive discharge summaries per regulatory requirements
• Document all required information in electronic documentation system
• Record treatment minutes accurately per regulatory requirements
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Attend required facility trainings, in-services and conferences
• Obtain necessary DME or supplies per regulatory requirements
• Supervise occupational therapist assistants, rehabilitation aides and students
• 1+ years of OT experience required
• LTC or Rehab experience required
• Active Oklahoma OT license required
• New graduates considered with Level II experience
• No on-call or weekend requirements
• Must adhere to NBCOT and State practice acts, code of conduct and ethics
