Job Details:

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
• Develop appropriate plans of care following regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Provide rehabilitative, skilled and medically necessary treatment interventions
• Administer various procedures including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive/assistive devices
• Assess patient progress and adjust treatments accordingly in clinical records
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers and other service providers
• Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in electronic documentation system and maintain patient records
• Accurately record treatment minutes for services provided
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Participate in facility and company required trainings, in-services and conferences
• Obtain necessary DME or supplies per regulatory requirements
• Supervise and support occupational therapist assistants, rehabilitation aides and/or students
• Maintain adherence to NBCOT and State Required practice acts, code of conduct, and ethics
• 1+ years of OT experience required
• LTC or Rehab experience required
• Active Oklahoma OT license required
• New graduates considered with Level II experience
• No on-call or weekend requirements
