PT - Physical Therapist

Job Details:

• Work Monday through Friday from 8:00 AM to 5:30 PM
• 40 guaranteed hours per week for 13-week contract
• Home health setting serving adult and geriatric patient populations
• Rotate one Saturday shift every 6-8 weeks with compensatory weekday off
• No holiday rotation required
• Occasional travel to central/northern regions with mileage reimbursement at government approved rate
• Productivity expectation of average 4.5 visits per day
• Contract includes 36 hours of low census unbillable hours eligibility
• Contractors must live outside 50-mile radius from facility
• Required certifications: AHA or ARC BLS, Wisconsin state licensure, NPI number
• Master's degree in Physical Therapy required
• Home health experience required with adult/geriatric population
• Experience with orthopedic, neurological, and functional mobility diagnoses
• Use laptop for point of service documentation with NetSmart EMR system
• Cell phone provided for GPS navigation and communication
• Dress code: scrubs (any color) or business casual
• Evaluate, treat, and plan patient care according to Physical Therapy standards
• Document to meet regulatory and reimbursement requirements
• Mask required at all times during flu season if declining flu and COVID vaccination
