Physical Therapist Assistant Job in San Ramon, CA

CVH Care Home Health is seeking a Physical Therapist Assistant for home health rehabilitation services in the San Ramon area. This is a 91-day travel position with a start date of May 4, 2026.

Role Overview

You will provide physical therapy services to patients in their homes across two service areas: Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) and Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City). You must be willing to travel outside these service areas occasionally and work weekends as needed.

Schedule and Compensation

Two schedule options are available:

• Guaranteed Hours: 36 hours per week with a minimum of 30 points per week productivity requirement
• Non-Guaranteed Hours: Per visit basis

Weekly pay is $1,752. Payment is calculated on a per visit basis, with routine visits equaling 1 point = 1.33 hours. Your rate covers drive/travel time, visit time, documentation time, and phone calls to patients and doctors.

Requirements

You must have a minimum of 1 year of professional Physical Therapy Assistant experience. Home health experience is preferred. You must be willing to see COVID-positive and COVID PUI patients.

Required Credentials

• Current California PTA License
• BLS certification
• Valid Driver's License
• Auto insurance and registration
• TB screening (annual)
• MMR, Varicella, and Hepatitis B vaccinations
• TDap (10 years)
• Flu vaccination (annual)
• Drug screen and physical examination
