Tide Health Group – The Oxford Rehab and Healthcare is seeking a Physical Therapist for a long-term care position in Haverhill, MA. This is a 91-day travel assignment working five 8-hour day shifts weekly at $2,448 per week, with a start date of June 15, 2026.

Your Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. You'll develop appropriate plans of care following all regulatory and clinical practice standards, utilizing standardized assessments and evidence-based practice to guide clinical interventions.

Key Responsibilities

  • Administer various procedures including manual techniques, ambulation training, therapeutic exercises, modalities, and assistive device instruction
  • Assess patient progress and adjust treatments accordingly, maintaining accurate clinical records per regulatory requirements
  • Evaluate treatment outcomes for effectiveness and modify services based on patient response
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
  • Provide comprehensive discharge summaries meeting regulatory and clinical requirements
  • Document all required information in the electronic documentation system and maintain patient records accurately
  • Record treatment minutes for services provided per regulatory requirements
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Attend facility and company required trainings, in-services, and conferences
  • Obtain necessary DME or supplies per regulatory requirements
  • Supervise and support physical therapist assistants, rehabilitation aides, and students

Requirements

  • Massachusetts State License required
  • 1 year of PT experience (new graduates acceptable with Level II experience)
  • Long-term care or rehabilitation experience required
  • Adherence to APTA and State Required practice acts, code of conduct, and ethics

This position offers no on-call or weekend requirements, providing consistent day-shift scheduling in a long-term care setting.
