Occupational Therapist Home Health Job in Fort Dodge, IA

You will provide occupational therapy services to patients in home health settings within a 50-mile radius of Fort Dodge, Iowa. As part of an interdisciplinary team, you'll contribute your clinical expertise to patient care and recovery planning.

Your Day-to-Day:

• See an average of 4–6 patients per day
• Work with patient populations requiring short-term therapy following hospitalization or nursing home stays
• Travel within the coverage area, averaging 200–250 miles of driving per week (mileage reimbursed)
• Attend weekly virtual team meetings and monthly in-person meetings
• Document and manage care using Epic EMR

Schedule and Duration:

This is a 91-day contract position with a 5x8 day shift schedule, starting July 13, 2026. No on-call or weekend requirements.

Orientation and Expectations:

Orientation begins every Monday and includes organization orientation, required online courses, 2 days of Epic EMR training, and ride-along/shadow visits. You are typically fully productive by week 3.

Requirements:

• Current Iowa state occupational therapy license
• BLS certification
• Minimum 2 years of previous OT experience; home health experience is preferred but not required
• Valid driver's license and your own vehicle (mileage reimbursed)
• Taxable address outside the 50-mile radius from the facility
• Epic EMR experience preferred
