Physical Therapist Job in Ada, OK

You will work as a Physical Therapist in a Long Term Care setting at Tide Health Group - Ada Care Center. This is a temporary position with a 91-day duration, beginning June 15, 2026.

About the Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care. Your daily responsibilities include:

• Develop appropriate plans of care following all regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Administer various rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments accordingly in clinical records per regulatory requirements
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate effectively with your supervisor and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records
• Accurately record treatment minutes for services provided per regulatory requirements
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Attend facility and company required trainings, in-services, and conferences
• Obtain necessary DME or supplies per regulatory requirements
• Supervise and support physical therapist assistants, rehabilitation aides, and students
• Adhere to APTA and State Required practice acts, code of conduct, and ethics

Schedule and Compensation

You will work five 8-hour shifts per week on days. Weekly pay is $2,390.40. There are no on-call or weekend responsibilities.

Requirements

You must have a minimum of 1 year of PT experience (new graduates with Level II experience are acceptable), Long Term Care or rehabilitation experience, and a current Oklahoma state license.
