Case Management RN Job in Fort Worth, TX

ADEX Healthcare Staffing is seeking an experienced RN Case Manager for a travel position at Medical City Fort Worth. This is a day shift role on the 5th Floor Neuro Step Down unit, working 40 hours per week from 8:00 AM to 4:30 PM with no on-call requirements.

Unit Overview

• 40-bed Neuro Step Down unit located in the main building
• Day shift team includes 1 RN and 1 Social Worker
• Patient ratio of 1:20–30
• Floating requirements vary based on case management assignment

Your Responsibilities

• Provide case management support for acute care patients
• Coordinate care transitions and discharge planning
• Document in Meditech and Careport EMR systems
• Collaborate with interdisciplinary team members
• Demonstrate strong problem-solving and self-directed initiative

Qualifications

• Current RN license
• Minimum 1 year of acute care case management experience
• Familiarity with Meditech and Careport preferred
• Certified Case Manager (CCM) designation preferred

• Duration: 91 days
• Start date: September 21, 2026
• Schedule: 5 days per week, 8-hour shifts
• Weekly pay: $1,694
