Physical Therapist Assistant Job in Rutland, VT

As a Physical Therapist Assistant with VNA Hospice of the Southwest Region, you will work under the direct supervision of a physical therapist to deliver skilled care to patients in the hospice setting. Your focus will be on implementing individualized treatment plans that prioritize functional independence, safety, and quality of life.

Your responsibilities will include:

• Carry out prescribed physical therapy interventions, including restorative and maintenance therapeutic techniques
• Assist in developing and refining plans of care with the physical therapist
• Assess patient status and progress at each visit, documenting findings and communicating changes in condition
• Conduct patient and caregiver education sessions to promote understanding of treatment goals and home management strategies
• Coordinate care with the clinical team, including physicians, nurses, caregivers, and other rehabilitation services
• Identify and report any changes in patient condition that may affect the prescribed plan of care
• Maintain thorough, compliant documentation in accordance with federal and state regulations
• Stay current with physical therapy standards and HHCAHPS quality measures
• Integrate quality standards into all visit protocols
• Adhere to organizational policies, APTA guidelines, and established standards of care

This is a 91-day travel assignment beginning April 14, 2026. You will work five 8-hour shifts per week on day schedule, with weekly compensation of $1,810.40. You will be part of a dedicated hospice team committed to compassionate, evidence-based care.
