Case Management RN Job in Denver, CO

HCA HealthONE Swedish in Denver is seeking an RN Case Manager for a 91-day travel assignment. You will work day shifts from 8:00 AM to 4:30 PM with weekend rotation coverage (Friday through Monday and Monday through Friday schedules). No night shifts are required.

About the Role

• Perform discharge planning assessments and coordinate discharge planning duties across patient populations
• Manage a patient ratio of 1:20 as an RN Case Manager
• Work with mobile equipment and computer systems to support care coordination
• Utilize Meditech and CarePort EMR systems
• Demonstrate strong communication skills and professionalism in all interactions
• Adapt to working across different units as needed

Requirements

• Current RN license
• Minimum 1 year of current Case Management RN experience required
• Discharge planning experience preferred
• Emergency department experience preferred
• Palliative care knowledge and goals of care conversation experience preferred
