PT - Physical Therapist

Job Details:

• Conduct skilled home health visits with a minimum of 6 visits per day (minimum of 6 points per day, may increase by 1 point at manager's discretion)
• Work with adult and geriatric patient populations including med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory patients, neuro, trachs, trauma, AKA, BKA, and ADLs
• Provide skilled home care services under Medicare, Medicare Advantage, and commercial insurance
• 1-2 weekend shifts per contract required
• Block scheduling type
• 1 year of experience required
• Must have valid driver's license and personal vehicle to conduct home visits
• Home health experience required; OASIS training preferred
• Required certifications: BLS and valid state license (FL license or compact license required)
• Required skills include: assessment, cardiopulmonary/circulatory, documentation, ortho, neuro, home exercise program, medications, patient and family education, safety and injury prevention, and billing
• Must be proficient with adaptive devices including gait belt, rolling walker, sit-to-stand lift, sliding boards, soft/hard collars, standard walker, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
• Equipment provided includes laptop, MiFi for internet access, and portable printers
• Charting completed using BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
• Cerner charting experience preferred
• Scrubs: Any solid color scrub required
