RN - Case Mgmt

Job Details:

• Monday through Friday schedule from 8:00 AM to 5:00 PM
• 13-week contract assignment
• No on-call requirements
• No weekend work required
• Patient ratio of 1:30 (RN to patient)
• Minimum 1 year bedside case management experience required
• Meditech or HCA experience required
• Post acute care knowledge required
• Throughput experience preferred
• Must be comfortable having difficult conversations with families, patients, and physicians
• Perform discharge planning and care progression
• Work with Meditech/Midas EMR systems
• Use computer and standard office equipment
• Dress code: Navy scrubs with white lab coat or business casual with white lab coat, closed toe shoes (no crocs or artificial nails)
• Must have strong personality and be adaptable and proactive
