Physical Therapist Job in Novato, CA

Join Sutter Health Care At Home Outer Bay Rowland Way as a Physical Therapist providing home-based rehabilitation services to adult and geriatric patients in the Novato area. This 91-day travel assignment runs 5 days per week on days shift, with a typical daily census of 5 patients.

Role Responsibilities:

• Conduct physical therapy evaluations, assessments, reassessments, and functional capacity examinations
• Develop individualized treatment protocols and home exercise programs based on patient needs
• Provide clinical interventions including gait training, extremity mobilization, spinal mobilization, and neurodevelopmental techniques
• Work with patients across orthopedic, neurological, and cardiopulmonary diagnoses
• Identify environmental barriers and educate patients and families on home safety modifications
• Complete OASIS documentation and maintain accurate plans of care
• Collaborate with interdisciplinary team members including physical therapy and social services
• Apply evidence-based practice to clinical decision-making and treatment planning

Required Qualifications:

• Valid California Physical Therapy license
• Minimum 1 year of clinical experience; 2+ years preferred
• Travel therapy experience required
• BLS certification (AHA or ARC)
• Valid driver's license and auto insurance
• EPIC electronic health record experience required
• Previous charge or supervisory experience preferred
