Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Provide physical therapy services in home health setting<br>• Coverage areas include Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville<br>• Additional travel outside assigned area may be required<br>• Weekend coverage may be required<br>• Guarantee option: 36 hours guaranteed per week with minimum 27 points productivity requirement<br>• Must complete 30 points per week to maintain guarantee<br>• Non-guarantee option: Payment made on per visit basis with no guaranteed hours<br>• Must be willing to see Covid+ and Covid PUI patients<br>• Visit types include OASIS certifications, evaluations, routine visits, discharges, and follow-ups<br>• Rate covers drive/travel time, visit time, documentation time, and phone calls to patients and doctors<br>• Required credentials: Current CA PT License, BLS, Driver&#39;s License, Auto Insurance, Auto Registration<br>• Required health screenings: TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical<br>• All requested time off must be disclosed at submission time</p><p><strong>Facility:</strong></p><p>This home health organization provides comprehensive rehabilitation and healthcare services directly to patients in their homes throughout multiple counties.
