About this role

Tide Health Group – Northwood Rehabilitation & Health Care Center is seeking a Physical Therapist for a long-term care setting in Lowell, MA. This is a temporary position working days, 5 days per week, 8 hours per shift, for 91 days starting June 22, 2026. Weekly pay is $2,563.20.

About the Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. You will develop and implement plans of care following all regulatory and clinical practice standards, utilizing standardized assessments and evidence-based practice to guide your clinical interventions.

Your responsibilities include:

  • Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and supportive and assistive devices
  • Assess patient progress and adjust treatments accordingly, documenting all findings in clinical records per regulatory requirements
  • Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Communicate with your supervisor and health team members regarding patient progress and treatment plans
  • Provide comprehensive discharge summaries per regulatory and clinical requirements
  • Maintain accurate documentation in the electronic documentation system and patient records
  • Record treatment minutes for services provided per regulatory requirements
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Supervise and support physical therapist assistants, rehabilitation aides, and students
  • Attend facility and company required trainings, in-services, and conferences

Requirements

You must hold a valid Massachusetts state license as a Physical Therapist. A minimum of 1 year of PT experience is required; new graduates with Level II experience are acceptable. Long-term care or rehabilitation experience is required. You must adhere to APTA and State Required practice acts, code of conduct, and ethics.

This position includes no on-call or weekend responsibilities.
