ADEX Healthcare Staffing is seeking a Physical Therapist for a 13-week travel assignment at Blue Oak Post Acute in Santa Rosa, California. This is an excellent opportunity to deliver rehabilitation services in a post-acute care environment while supporting patients through their recovery journey.

Assignment Details:

• 13-week duration
• Rotating shift schedule
• 30 hours guaranteed per week
• Start date: June 1, 2026
• Weekly pay: $2,774

Your Role:

As a Physical Therapist, you will provide direct patient care in a post-acute rehabilitation setting. You will work with patients in various stages of recovery, developing individualized treatment plans to support their return to function and independence. Your responsibilities will include:

• Conducting patient assessments and evaluating rehabilitation needs
• Developing and implementing evidence-based treatment plans
• Providing hands-on therapeutic interventions and patient education
• Documenting patient progress and clinical outcomes
• Collaborating with interdisciplinary healthcare team members including nurses, occupational therapists, and physicians
• Coordinating care to ensure continuity and optimize patient outcomes

About You:

You hold a degree in Physical Therapy and are licensed to practice in California. You have experience working with patients in rehabilitation and recovery phases, and you are comfortable in a rotating shift environment. You communicate clearly with patients and team members, and you take pride in comprehensive documentation and coordinated care delivery.
