SLP Job in Moore, OK

Tide Health Group - Thunder Care and Rehab is seeking a Speech-Language Pathologist for a long-term care assignment in Moore, Oklahoma. This is a 91-day position running 5 days per week, 8-hour day shifts, with no on-call or weekend requirements.

Your responsibilities will include:

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
• Develop plans of care following regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Assess patient progress and adjust treatments accordingly in the clinical record
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Make referrals in relation to treatment outcomes
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders
• Communicate effectively with supervisors and health team members about patient progress, barriers, and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records per company policy
• Accurately record treatment minutes for services provided
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Participate in facility and company required trainings, in-services, and conferences
• Obtain necessary DME or supplies per regulatory and clinical practice requirements
• Supervise and support CFY or student speech-language pathologists as needed
• Adhere to ASHA and state required practice acts, code of conduct, and ethics

Requirements: Current CCC certification, active ASHA membership, and Oklahoma state licensure required. Minimum 1 year of SLP experience and long-term care or rehabilitation experience required.

Compensation: $2,534.40 per week. Start date: July 6, 2026.
