PT - Physical Therapist

Job Details:

• Work in Acute and Extended Long-term Care (ELC) settings
• Full-time position requiring 40 hours per week
• Schedule needs to be reviewed during interview with rehabilitation manager
• Float between acute/swing beds, nursing home, and outpatient services as indicated
• Provide treatment in patient rooms, hallways, outdoor areas, and rehabilitation gym
• Perform physical therapy evaluations and establish plans of care
• Assist with rounds and care conferences as indicated
• Maintain frequent communication with rehabilitation team regarding patient progress, equipment needs, and discharge planning
• Prefer clinician with at least 3 years of experience in medical setting
• Must have experience in hospital and/or nursing home setting
• Required certifications: CPR and North Carolina Physical Therapy license
• Dress code: Business casual or scrubs
• Documentation systems: Cerner and NetHealth
• Patient ratios: 1:1
• Access to typical rehabilitation equipment and both hospital and nursing home gyms
