Physical Therapist Job in Honakaa, HI

Join Honokaa Hospital & Skilled Nursing as a Physical Therapist on a 91-day travel assignment starting July 27, 2026. You'll work a rotating schedule, typically Tuesday through Saturday, with possible weekend coverage as needed to support 24/7 facility operations.

In this role, you will:

• Perform diagnostic assessments and develop individualized treatment plans
• Deliver professional physical therapy services for both habilitation and rehabilitation
• Work primarily with skilled nursing facility and inpatient cases, with some outpatient services
• Serve the full range of cases within your program scope
• Manage all therapy services for your assigned unit, center, or facility as needed
• Use the Meditech EMR system for documentation and care coordination

Requirements:

• Minimum 6 months of work experience in at least one setting: acute care, skilled nursing facility, rehabilitation hospital, or outpatient
• Experience working with adult patients
• Current BLS certification
• Hawaii state Physical Therapist license required by assignment start date
• Ability to arrive Thursday before your start date for Friday employee health appointment

This assignment offers you $2,277.60 weekly pay and the opportunity to deliver meaningful therapy services in a diversified care setting. You'll gain exposure to both acute rehabilitation and long-term care populations while working with an established healthcare team.
