Opportunity:
PT - Physical Therapist
Job Details:
• Provide physical therapy services to patients in subacute and skilled nursing setting
• Work with patients requiring rehabilitation and recovery services
• Develop and implement individualized treatment plans
• Document patient progress and maintain accurate records
• Collaborate with interdisciplinary healthcare team
• Must maintain current Physical Therapist license and certifications
• Experience in skilled nursing or subacute care preferred
