Physical Therapist Job in Louisville, KY

ADEX Healthcare Staffing is seeking a Physical Therapist for a 91-week travel assignment with VNA Health at Home—Louisville Metro, beginning June 22, 2026.

Role Overview

You will deliver home health physical therapy services across the Louisville Metro service area. Your responsibilities include conducting start-of-care evaluations, recertifications, and daily patient visits. You'll work with common diagnoses including congestive heart failure, orthopedic conditions, and pneumonia.

Schedule & Compensation

Monday through Friday, 8:00 AM to 5:00 PM with rotating weekend participation. Workdays are 5–8 hours with a requirement of 6.5 points per day or week. You'll earn $2,482 weekly.

What You'll Need

• Current BLS certification
• Physical Therapy degree
• Home health experience preferred but not required for new graduates

What's Provided

• Two weeks of orientation with supervised visits alongside a workcoach
• Phone and tablet for field work
• 24/7 clinical manager or educator support for emergencies; 8:00 AM–5:00 PM support for general questions
• Homecare Homebase electronic charting system

Work Environment

Dress code is scrubs (any color) or business casual. You'll participate in the facility's holiday rotation schedule. The assignment allows for up to four shift cancellations per 13-week period, or three cancellations for shorter assignments.
