Job Details:

• Minimum Bachelor's Degree in Physical Therapy from a school accredited by the American Physical Therapy Association (APTA) required
• Current Wisconsin licensure to practice Physical Therapy required
• Current BLS certification required
• Clinical Specialist certification beneficial
• Valid Driver's License required
• Automobile liability insurance with $100,000/$300,000 coverage required, or $300,000 combined single-limit liability
• Provide home-based physical therapy services to patients
• Travel to patient homes to deliver therapeutic care
